For those that have never experienced hospice personally, they may not have a full appreciation for the extensive range of services we offer and our support for patients AND caregivers. However, for many, even the word “hospice” can be a challenging topic. We have developed a special program to help provide an introduction to the collection of services – even if a patient is not quite ready.

Palliative Pathways

Our Palliative Pathways support team will provide support, education, guidance and community resources as appropriate for your unique situation.

Palliative Care is a philosophy of care that is often referred to as supportive care for treatment of symptoms associated with a serious illness, in addition to psychological, social and spiritual concerns. The goal for palliative care is to align a patients treatment with their individual goals, expectations and beliefs. Hospice of the West has a dedicated Palliative Care team to support individuals who have a serious illness but not yet ready for hospice, who have been discharged from hospice and those who need extra support while receiving specialized medical treatment.

When patients are facing a serious illness, Hospice of the West Palliative Pathways Program offers services that address the most pressing needs today, while also planning for tomorrow. Our physicians and advanced practice providers help patients, their caregivers, along with their primary physician, manage pain and other distressing symptoms that can make each day a challenge. We also connect those who are facing a chronic and or/terminal condition with education, support, and community resources.

The Palliative Pathways Program is provided by Hospice of the West, at the request of a patient’s health care provider, to collaborate with the patient’s health care team. This outreach program is appropriate for patients with a debilitating disease, chronic illness and life threatening illness.
